Explicit Consent Form for Processing Personal Data

In accordance with the “Personal Data Protection Law” No. 6698, I have been informed within the company regarding the relevant legal regulations and the rights I possess within this framework. I have read and reviewed the clarification text provided to me. Under the relevant laws, I consent to the processing of my necessary personal data, special category personal data, and health data, which I have provided verbally, in writing, and/or electronically, including those identifying or helping to identify me, by “EMAN SAĞLIK HİZMETLERİ MEDİKAL TİC. LTD. ŞTİ.” for the purpose of delivering the required service and planning these services as per the work performed, and sharing with other persons and organizations where such sharing is required under relevant legislation. I consent to the processing of my personal data, special category personal data, and health data within the scope defined in the “Personal Data Protection Law” No. 6698, as detailed below.

Personal data that clearly pertains to an identified or identifiable real person, processed partially or fully automatically, or non-automatically as part of a data recording system, including:

• Identity information such as name-surname, T.C. identification number, nationality information, mother’s name-father’s name, place of birth, date of birth, gender, including documents like driver’s license, ID card, and passport; Judicial Records, Diploma Copies, Disability Certificate if needed, Military Service Status Certificate, Health Report, Blood Group Card, Biometric Data, Marriage Certificate if applicable, Tax Number, Social Security Number, Credit Card Information, Bank Account Information, Signature Information, Vehicle Plate, Names and Surnames of Spouse and Children, T.C. Identification Numbers, contact information of a relative to be reached in emergencies including name, surname, and phone number.
• Contact information such as phone number, address, email address, fax number, IP address.
• Marketing data including shopping history, surveys, cookie records, and information obtained through campaign activities.
• Data obtained through record-keeping for security and workflow purposes, such as card or cardless entry system records, camera recordings, records taken at security points, vehicle tracking system records if a company vehicle is used, and access logs related to internal computer usage.
• Personal data related to any requests or complaints directed to “EMAN SAĞLIK HİZMETLERİ MEDİKAL TİC. LTD. ŞTİ.”

I have read the Clarification Text about the Protection and Processing of Personal Data provided by “EMAN SAĞLIK HİZMETLERİ MEDİKAL TİC. LTD. ŞTİ.” on their website, in printed & electronic form, or sent via SMS to the communication number I declared as my contact number. I accept, with my free will and the approval code I have verbally stated, the recording, storing, preservation, reorganization of my personal data, and sharing with institutions legally authorized to request such personal data as described above.
